Having headed up a busy operational side of business and routinely looked at root causes from minor safety incidents, we used this procedure to look at other issues in the business where we had gone off track.
What we found was surprising. 80% of the issues had their roots in early stage design.
We then looked to improve our design processes, visibility and flow. This made a difference to the operational impacted work and a lot of the design process remedies were undertaken on the desktop, low cost and with no time impact on programme.
